If you do not desire to have a  Free Discovery Call, please fill out this form to request your online forms for Step 2.

Your comprehensive membership package includes all of the following:

• extensive review of your medical records
• 60-minute Initial Consultation with medical provider with lab recommendations
• review of all data from your lab results
• 60-minute Treatment Plan appointment with medical provider with recommendations
• 90-minute initial consultation with our certified Health Coach 
• 6 additional weekly or bi-weekly 45-minute sessions with the Health Coach
• 30-minute Follow-up appointment with medical provider

This package totals $1995 and is due at the time of booking your Initial Consultation appointment. 

You will receive two emails each with unique links to fill out your forms: one from Starlight Wellness and one from FM Logics. These forms are very comprehensive and will take at least an hour to complete. Once we have received your completed forms and received your past medical records, our office will call to schedule your Initial Consultation.

A link for a HIPPA-compliant Zoom meeting will be sent to you prior to your consultation appointment. This appointment will be 60 minutes. Our provider will have reviewed your comprehensive health history forms and all pertinent medical history/labs performed over the last two years. A recommendation will be made at that time for further advanced functional medicine tests. 

Have all the labs completed which were recommended by our provider at your Initial Consultation. These labs can be completed at your home or through a 3rd party lab. The cost for any laboratory testing  is billed separately through Rupa Health, our lab platform. 

Once the labs are reviewed by our provider, you will be contacted to schedule a Treatment Plan telemedicine appointment. Our provider will make recommendations for nutritional and life-style changes, therapies, and/or supplements. In some cases, pharmaceuticals will be prescribed based on all the information gathered. Please plan on a 60-minute telemedicine visit.  

Our provider will then collaborate with our certified Health Coach regarding your treatment plan. She will reach out to you to schedule your initial Health Coaching session. She will provide personal support and resources to help facilitate your journey to optimal health. We believe that health coaching is the key to unlock your true potential, whether you're looking to improve your fitness, nutrition, mental health, or overall well-being. She will then set up weekly or bi-weekly health coaching sessions for you, teaching you how to nourish your mind, body, and soul.  

The Follow-up appointment with the provider will be a 30 minute telemedicine visit. You will be sent Follow-Up forms to complete 2 weeks prior to this visit. These forms must be completed and returned within 1 week of your Follow-up appointment to allow for proper review before you can be seen. 

At the Follow-up appointment, our provider will review your progress and proceed with any further recommendations of lifestyle, nutritional changes, as well as further testing or treatment options. This applies to all further Follow-up appointments.

The first Follow-up appointment is included in your 3-month Membership Package. All subsequent Follow-up appointments are $200 per appointment. The fee is due at the time the appointment is scheduled. If it has been over 2 years since you were last seen we will need to start with an Initial Consultation.

Cancellations/Refunds: If you cancel your Initial Consultation appointment after you have paid your fee, you will be refunded 75% if done at least 24 hours prior to appointment date/time. This is due to the extensive nature of medical review prior to your appointment.  There are no refunds if an Initial Consultation appointment is cancelled within 24 hours of appointment date/time. There are no refunds after you have completed the Initial Consultation appointment with the provider. 

Rescheduling Appointments: If you reschedule at least 24 hours prior to your scheduled appointment date/time, there is no penalty or further fees rendered. If you reschedule within 24 hours of your scheduled appointment date/time, $50 fee will be assessed.
