Welcome to Starlight Wellness Functional Medicine!
You have made a good decision.
To get started the following need to be completed:
1. Complete online functional medicine questionnaire.
The questionnaire will be sent in a separate email.
Please note that this questionnaire will come from "FMLogics" and the subject line will read:
"A Questionnaire from Your Clinician"
***Please be as thorough and as detailed as possible when completing your medical online questionnaire. Please DO NOT SKIP any portion of this comprehensive questionnaire. The more information you can provide the better Julie Loyd, PA-C can gain a more complete overview of your health issues allowing him to determine what is at the root of your illness.
2. Activate Your Hint Health Portal
Our HINT HEALTH portal is an HIPAA protected electronic medical record.
Your health information is protected and kept strictly confidential between Starlight Wellness and you.
A form of payment is required during sign up but will not be charged until your appointment is scheduled.
We cannot move forward until you activate your HINT HEALTH PORTAL
- Once your portal has been established, the email from FMLogics will be sent to your inbox.
- We also need access to your current patient portals from other institutions to review your medical records.
- There will be a form for you to fill out your patient portal URL and login information from your present physicians and specialist via our HINT HEALTH portal.
- The reason Starlight Wellness must have access to your patient's medical portal is because countless times medical items have been left out or overlooked leading to an incomplete history. This makes it difficult or impossible for Julie Loyd, PA-C to do her due diligence in her clinical workup of a patient's case.
- She wants to make sure nothing is missed.
Unfortunately Starlight Wellness will not make any exceptions to having access to your Patient Portals.

3. Schedule Your Consultation
Once you have completed the functional medicine questionnaire, and forms through HINT HEALTH, you will be called to schedule an in-person appointment in our Grand Rapids office or a virtual consultation with Julie Loyd, PA-C. The consultation will be approximately 90 minutes.
Your method of payment on file will be charged at the time you schedule your consultation.
